Pursuing National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognition is the most widely used way to transform primary care practices into Patient-Centered Medical Homes.
Based on the collaborative report, Joint Principles of the Patient-Centered Medical Home, a Patient-Centered Medical Home or PCMH is a team based health care delivery model led by a Physician, Physician’s Assistant, or Nurse Practitioner that provides comprehensive and continuous medical care to patients and coordinates treatments across the health care system.
